With Momentum, you choose the plan that best fits your needs and your budget. Our Gold Plan, outlined here, is our intermediate level plan — more coverage than the Protect Plan, but not as comprehensive as the Platinum Plan.
Gold Plan members have an annual maximum benefit of $1,200 per person, per benefit year.
A $50 deductible applies to single coverage for each individual in a two-person plan. For family plans the maximum deductible is $150 for the entire family. The deductible is an annual deductible based on the 12 consecutive months beginning with the effective date of the plan.
Waiting periods may be waived if you were covered under an employer-sponsored group policy within 60 days of the start of your coverage under this policy, or an individual policy within 30 days of the start of your coverage under this policy.
Momentum Platinum and Gold Plans do not cover oral surgery, implants, orthodontics, or cosmetic services.
