      Individual Dental Insurance FAQs

      Anyone in Wisconsin can qualify for individual dental insurance from Momentum Plans. There are many reasons why you may not have dental insurance coverage. If your current employer does not offer dental insurance, if you do not qualify for your company’s dental plan, if you are currently unemployed, or if you are retired you can continue to receive dental healthcare in Wisconsin with individual dental insurance.

      Many individuals do not realize their dental health is not covered by most Wisconsin employee healthcare plans. As a result, many people fall behind on their oral health until it is too late. With individual dental insurance, you can continue to receive the best oral hygiene treatments and preventive services in Wisconsin.

      If your current provider is a part of the Momentum Plan network of premier dentists, you may continue seeing your current provider.       The dentists who participate with Momentum have been ensured to meet national credentialing standards.

      Comprehensive individual dental plans cover everything from regular cleanings to full mouth or panoramic x-rays. Children on an individual dental plan can receive annual fluoride treatment, space maintainers (through age 15) and sealants (through age 13). From fillings to crowns to dentures, your will have complete oral health coverage through Momentum Plans. Root canal therapy and emergency treatment is also partially covered when necessary.

      If you require special care, the Gold or Platinum dental insurance plans may be the best option for you. The Single Rate, Two-person Family, and Family Rate are available for both levels of coverage. The Family Rate covers up to four family members, with additional member coverage available for additional fees to meet the needs of your family.

      Group Dental Insurance FAQs

      The dentists who participate with Momentum go through a credentialing process prior to joining the network and a review process on a routine basis.

      Our group insurance plans are flexible and affordable, covering a wide range of dental services and needs. Depending on the plan that you choose, it may offer coverage ranging from basic services and preventative care to restorative and other specialized services.

      Employer Sponsored is when the employer pays for a portion of the dental benefit. Voluntary is when the employee pays for 100% of the dental benefit.

      If their current provider is a part of our network of premier dentists, they will not have to change their current dental care provider. If their current dentist is not part of our network, they will be able to choose another provider meeting their needs from our network. Additionally, you can decide to add an out-of-network benefit to your policy. Our network includes a significant number of dentists across Wisconsin.       Yes. All of our plans are customizable to treat any number of dental procedures.

      Yes. Spouses and children (up to the age of 26) are covered by our flexible, customizable insurance plans.

      Momentum insurance plans exclude the following:

      • Services or prosthetics started before the date of coverage eligibility.
      • Dental procedures treating injuries or diseases caused by or related to riots or any other form of civil disobedience.
      • Dental procedures treating injuries sustained during a criminal act.
      • Dental procedures treating deliberate self-inflicted injuries.

      Yes. Emergency treatment is covered both inside and outside of the Momentum Service Area, except for treatment needed to address injuries or disease resulting from civil disobedience or criminal activity.

      You may choose the dentist of your choice. However, depending on your plan you may be responsible for some or all of the costs. Some employer plans include out-of-network benefits, while others do not. Check your plan details or contact us to verify coverage.

      Yes, you can ask your dentist to apply to the network.

      After a dentist visit, you will receive a document in the mail that lists the dental services you received during your treatment. This is not a bill, but rather a breakdown of the services you received and their total costs. The EOB will show what portion of these services was covered by your insurance. Although EOB’s may vary between companies and types of insurance, it should include the dentist’s fees, the portion your insurance paid and the amount you may owe (such as deductible, coinsurance, or non-covered services).
